There has never been a Presidential candidate in the history of the U.S. with Trump's record of adjudicated criminal guilt (sexual assault, illegally influencing a New York political campaign, racial discrimination in housing, and fraud in Trump University), not to mention his trail of bankruptcies. You may certainly disagree (as did the majority of voters in the 2024 election ), but it is very reasonable - not deranged - to declare Trump unfit for office.
TDS does not present primarily as disapproval of the Orange Man but by the obdurate resistance to any policy that is good for the country but which, if adopted, might somehow, by someone, be interpreted as vindication of an idea associated, however remotely, with him.
